Geriatric Evaluation and Management Program (GEM)
The Geriatric Evaluation and Management Program offers a one-of-a-kind comprehensive assessment provided by a board-certified geriatrician and a geriatric social worker. The three-hour evaluation — which includes a bio-psychosocial assessment, screenings for memory, depression, nutrition, and general functioning — also includes a full exam by one of our geriatricians. The office visit concludes with a patient and family conference where members of the GEM team share their findings and develop an individual care plan. In addition to medical care, we provide patients with referrals to and assistance with medical and community resources to address the identified issues.
