Crozer-Keystone Home Health Ranked Among Top 25 Percent Nationwide for Ninth Time

HomeCare Elite is an annual recognition of the top performing Medicare-certified home health agencies in the United States.

Crozer-Keystone’s Home Health department was recently named to the “HomeCare Elite,” ranking it among the top 25 percent of Medicare-certified home health agencies in the country. 2016 marks the ninth time in 11 years that Crozer-Keystone Home Health was awarded this designation. Winners are selected using publicly available data from Home Health Compare and the Centers for Medicare & Medicare Services (CMS) Cost Reports. Out of 9,406 agencies considered, 2,353 were named “elite” based on a cumulative analysis of performance measures in quality of care, quality improvement and consistency, patient experience, process measure implementation, and financial performance. This annual listing of the nation’s top-performing home care providers is compiled by DecisionHealth, a publisher of healthcare business and regulatory news and analysis, and Ability® Network, a leading healthcare information technology company.

“I’m very proud of the Home Health staff,” says Jane Hanahan, R.N., B.S.N., M.H.A., administrative director of Crozer-Keystone Home Health and Hospice. “We have a great team with many years of service – most for over 15 years or longer – and we work extremely well together. For patients, this is verification that we provide outstanding home care services. Patients have a choice, so when they choose us they know they are getting an elite home care agency – one of the best in the nation time and again. With consistency comes quality.”

Hanahan adds that her staff strives to provide the best patient care by coordinating efforts with all levels of providers – from primary care physicians to specialists, nurse practitioners and nursing homes. The goal is to prevent re-hospitalization and keep patients as healthy as possible.

Quality is a top priority. Hanahan notes that Crozer-Keystone Home Health offers key patient-satisfier services, such as medication reconciliation, evening hours, and 24/7 nursing and physical therapy on-call. “Not many agencies have therapists who work on weekends,” she says. “In addition, we keep up with the latest technologies in wound care, have two wound care nurses on staff, and offer best practices and treatments for congestive heart failure, diabetes and joint replacements.”

The approximately 125 employees in the Crozer-Keystone Home Health department support about 600 daily cases on average through the “elite” service, which has been serving patients in Delaware County for nearly 30 years. “It’s great being a local provider,” Hanahan says. “The staff is proud to work here and enjoy serving the community they live, work, and in many cases, have grown up in.”
