Crozer-Keystone Launches Tailored Services for Seniors at Crozer-Chester Medical Center Emergency Department

Crozer-Keystone Health System has launched a new senior services program at the Crozer-Chester Medical Center Emergency Department (ED) to improve patient experience and bridge the gap between the care given in the ED and additional services needed for the senior population.

The ED will identify senior patients upon arrival and work quickly to move the patient into a customized room to see the physician. Several patient exam rooms have been designated in the ED for senior care and have been equipped with larger clocks and televisions and softer lighting.

In addition, Anumeet Cheema, MSN, RN joined Crozer-Keystone in December in the newly created position of Geriatric Coordinator. As a geriatric coordinator, Cheema will work to ensure patients over 65 years of age who visit the ED of Crozer-Chester Medical Center are scheduled for all necessary follow-up appointments and are sent home to a safe environment conducive to their continued care.

Cheema reviews each discharge plan and provides a clinical and social assessment of their current situation. She will also assist with such tasks as scheduling necessary follow-up appointments for testing or with specialists, and locating a primary care provider, if a patient should need one. In addition, Cheema collaborates with staff from Crozer-Keystone's Senior Health Services department to provide patients with referrals to community-based agencies and services within Crozer-Keystone.

This is not a one-time involvement with patients. She will check in with patients to ensure that the appointed follow-up care has been completed.

“Having an ED Geriatric Coordinator will enhance patient care for the senior population. With the coordinator’s help, care becomes more integrated and preventable repeat visits to the ED are likely to be avoided,” says Bruce Nisbet, M.D., chair of the Emergency Department at Crozer-Chester Medical Center.

This program is a part of the Crozer-Keystone Center for Geriatric Medicine under the direction of William S. Zirker, M.D., chief of Geriatrics for Crozer-Chester Medical Center, Springfield Hospital and Taylor Hospital, and medical director of Crozer-Keystone Senior Health Services.
