Home Care Department Earns ‘HomeCare Elite™’ Status for Fifth Year

For the fifth year in a row, Crozer-Keystone’s Home Care department was selected as one of the “2011 HomeCare Elite™,” the annual compilation of the most successful home health care providers in the U.S. 

Organizations named to this status are ranked based on performance measures in quality outcomes, quality improvements and financial performance. The information used to measure each of the three areas for this annual review, which identifies the top 25 percent of agencies in the nation, is based on publicly available data that were submitted by each home health agency. 

“The nursing staff, therapists, aides and management team are all responsible for the great care given to our patients,” says Jane Hanahan, administrative director of Home Care and Hospice for Crozer-Keystone Health System. “We work to provide quality care to our patients while being fiscally responsible, which is a major challenge. The ‘HomeCare Elite™’ status demonstrates our commitment to providing quality care to our patients. I am proud that we have been given this distinction for the fifth year in a row and I look forward to meeting the same goal in the years to come.” 

The re-hospitalization rates of Crozer-Keystone’s Home Care Department are among the lowest in the country. “With a rate of 21 percent, in comparison to the national rate of 29 percent, our department has made this initiative one of our main goals,” Hanahan says. “These rates prove that we are leaders in the prevention of re-hospitalization and work hard to keep our patients healthy.”
