Patient-Centered Medical Home

The National Committee for Quality Assurance (NCQA) recognizes Crozer-Keystone Health Network’s primary care practices as Level III Patient-Centered Medical Homes (PCMH). This is the NCQA’s highest designation for Medical Home Practices. To earn this certification, practices must undergo a rigorous set of standards set forth by the National Committee for Quality Assurance.

Studies have demonstrated that following the medical home model can improve patient access and help to reduce medical costs, such as unnecessary admissions and emergency department visits.
