Patient-Centered Medical Home
Crozer-Keystone believes in the “patient-centered medical home” philosophy of primary care. Many of the Crozer-Keystone Health Network practices have earned Recognition from the National Committee for Quality Assurance as Patient-Centered Medical Homes.
A patient-centered medical home is a team-based model of care led by a personal physician who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes. The patient-centered medical home is intended to result in more personalized, coordinated, effective and efficient care that uses the latest guidelines for quality and patient safety and utilizes the latest information systems technologies. ChesPenn Health Services
Since 1995, Crozer-Keystone Health System (CKHS) has provided oversight and technical assistance for ChesPenn Health Services. ChesPenn Health Services is a federally funded health center that provides low-cost health care services to the people of Delaware County. Serving more than 12,000 patients each year, ChesPenn is committed to its mission of providing quality medical and dental services to members of the community.
