Home Care and Hospice
Crozer-Keystone's Home Care program provides care in the comfort of patients’ homes. These patients may be newly discharged from the hospital, referred directly by physicians’ offices or in need of skilled care.
Health care and supportive skilled services are provided through a team approach that incorporates the quality care that has been a Crozer-Keystone tradition. The Home Health team, a group of highly skilled professionals, works in cooperation with patients’ primary care physicians to formulate an individualized plan for care.
In addition, Crozer-Keystone offers hospice care and support for terminally ill people and their loved ones. Hospice philosophy encourages the continuation of a meaningful life in the presence of a terminal illness. The Hospice Program can provide physical, emotional, social and spiritual support to Hospice patients and their families/caregivers.
The Hospice Interdisciplinary team consists of nurses, a social worker, clergy, home health aides, volunteer and therapists.
