Psychiatric Rehabilitation

Social Steps Psychiatric Rehabilitation helps adults 18 years and older who have functional disabilities from mental illness to develop, enhance and/or retain psychiatric stability, social competencies and personal adjustment. The program also teaches social skills so that clients can experience more success and satisfaction in the environment of their choice. Participants often advance to the point of readiness for a more demanding structured growth-oriented program and will be encouraged to do so. 

Services

Individuals develop a planned program of goal-setting, functional assessment, identification of needed and preferred skills and supports, education, and managing supports and resources to help produce the desired outcome. The program may include skill teaching four days per week, health education and psycho-education once per week and community integration twice a month, depending on the individual’s goals. 

Eligibility

The psychiatric rehabilitation assessor for Delaware County completes an assessment packet that determines specific supports or resources the individual needs and the skills and goals the individual prefers to develop, achieve and maintain. 

Duration of services

Services are available for as long as they are needed. Individuals can voluntarily terminate from the program. Upon discharge termination, clients are informed of their rights and the appeal process.
