SPRINGFIELD, PA – Crozer-Keystone Health System’s Home Health and Hospice Department has moved its headquarters from Drexel Hill to 200 West Sproul Road, Springfield.

Crozer-Keystone Home Health and Hospice’s mission is to offer a full range of home health and hospice care to the communities it serves. As a nationally recognized, seven-time member of HomeCare Elite™, Crozer-Keystone’s home health and hospice services meet the highest quality standards for patient care.

Home health professionals provide care in the comfort of patients’ own homes. Services provided include skilled nursing care, home health aides, rehabilitative therapies, medical social services, family education, pain management, and enterostomal therapy. Crozer-Keystone also has certified wound specialists who visit patients at home to help with the care plan. In addition, the team includes registered nurses who provide special mother-baby care to recently discharged mothers and infants. Health professionals will work with physicians to create a personalized care plan to make sure patients stay on track for recovery.

Hospice care professionals offer personalized, compassionate care to patients and their loved ones. Hospice care minimizes pain and provides comfort, and can be provided at home, in an assisted living facility, nursing home or in Crozer-Keystone’s inpatient Hospice Residence, located at Taylor Hospital. The Crozer-Keystone Hospice Program also has a bereavement support group open to all adults coping with the recent loss of a loved one.
