Health System Now Offers Navigators in Two Emergency Departments

Crozer-Keystone welcomes Patient Navigator Catherine Ray to the Emergency Department (ED) of Crozer-Chester Medical Center.

Crozer-Keystone Health System recently welcomed Catherine Ray to the Emergency Department (ED) of Crozer-Chester Medical Center in the newly created position of Patient Navigator. This unique service, which is also offered in the ED at Delaware County Memorial Hospital, helps to bridge the gap between the care given in the ED and services needed afterward.

As a patient navigator, Ray will assist patients with such tasks as scheduling necessary follow-up appointments for testing or with specialists, and locating a primary care provider, if a patient should need one. Ray will not have a one-time involvement with patients. She will check in with patients to ensure that the appointed follow-up care has been completed.

“Having an ED Patient Navigator will enhance care and follow-up for our patients, whether a patient has insurance or not. With the navigator’s help, care becomes more integrated and preventable repeat visits to the Emergency Department are likely to be avoided,” says Bruce Nisbet, M.D., chair of the Emergency Department at Crozer-Chester Medical Center.

A native of Delaware County, Ray brings an extensive background and expertise in medical office scheduling, electronic medical records and health insurance coordination.

Patient navigation services have been offered in the DCMH ED, to great success, since earlier in 2015.
