Information About Crozer ChesPenn Patient Information Incident

Notice Regarding Incident for Crozer-Keystone Health System Patients

Crozer-Keystone Health System (CKHS) is committed to protecting the confidentiality of patient information entrusted to us. This notice concerns an incident involving some of that information.

On June 4, 2013, we were informed by law enforcement that two employees improperly acquired patient information in order to assist in the preparation of fraudulent tax returns. As soon as we learned this, we immediately terminated the employees and began an investigation to determine the information involved. Specifically, the former employees may have obtained patient information, including patient names, addresses, telephone numbers, dates of birth, Social Security numbers, and in some instances clinical and health insurance information. Patient medical records were not taken and this incident will not affect patient medical care. We are actively cooperating with law enforcement in furtherance of their investigation. This incident does not affect all CKHS patients, only a small number of patients who were seen in the Crozer-Chester Medical Center between July 2011 and November 2011. 

We began mailing letters to affected patients on August 2, 2013 and have established a dedicated call center for patients to call with any questions. If you believe you are affected but have not received a letter by August 23, 2013, please call 1-877-218-0056 Monday through Friday between 9:00 a.m. and 7:00 p.m. Eastern Time and say the following telephone passcode 6622080113 when prompted.

We deeply regret the inconvenience to our patients. To help prevent something like this from happening in the future, we have we have re-educated employees regarding the importance of handling patient information.
