How Has the Road to Wellness Initiative Helped You on Your Healthy Journey?

Have the results of your biometric screenings encouraged you to eat healthier food, become more active, strive for your ideal weight, practice stress management, stop smoking or partner with a Wellness Works Nurse Navigator? Celebrate your journey on the Crozer-Keystone employee Road to Wellness by sharing your success story.

It’s been one year since Wellness Works first offered biometric screenings as part of the Road to Wellness initiative. More than 2,800 (46 percent) of eligible employees and spouses participated in biometric screenings last year. 

If you participated in any aspect of the Road to Wellness — the Know Your Numbers biometric screenings, online Health Assessment Questionnaire, or partnered with a Wellness Works Nurse Navigator — we want to hear from you. Just send your story (200 words or less) by Apr. 15 (Tax Day) to Susan Winkles, Employee Benefit Services, Taylor Hospital, via email at (susan.winkles@crozer.org), fax (19-6380 or 610-595-6380) or internal mail. 

The three most compelling success stories will be selected, earning recognition for the steps you’ve taken to improve your health and/or quality of life:

  • First Prize: $100 American Express Gift Cheque
  • Second Prize: $50 American Express Gift Cheque
  • Third Prize: $25 American Express Gift Cheque

Plus, all three receive a Road to Wellness t-shirt.

With your permission, we’d also like to publish your story in upcoming Wellness communications as an incentive for your coworkers.

Look for information about the steps that eligible employees have to complete to qualify for lower 2015 Wellness Medical Rates coming to you via email and in your home mail box this spring. Don’t miss out on an opportunity to take charge of your health and to receive financial rewards.  As always, participation is voluntary and confidentiality is guaranteed.  Eligibility for Road to Wellness programs and incentives is determined by Collective Bargaining Agreements.
