The “Road to Wellness” for Employees Now Has Nurse Navigators

Wellness Works’ “Road to Wellness” program recently welcomed two nurse navigators. The navigators are personal advocates who educate, guide and support participants in the CKHS Advantage, Advantage Plus and Indemnity medical plans in achieving health goals and finding the right care at the right time.
Cathy Petrucci, R.N., and Delores Pascale, R.N., are no strangers to the Crozer-Keystone Health System. Petrucci has been with Crozer-Chester Medical Center since 1995 and has worked in various nursing positions throughout the hospital. She earned her B.S.N. from the University of Delaware and her M.S.N. in Nursing Education at Wilmington University.
“I am honored to be part of this new program to provide personalized health maintenance support for our own employees. The navigator program allows us to serve as your guides from the inside, to coordinate your life with your health care, and your health care with your life. The overall mission is to help employees be well, get well, and stay well,” Petrucci says.
Pascale has been with the health system since 1998, most recently working at Delaware County Memorial Hospital. She is a graduate of Delaware County Community College with a degree in Applied Science, Nursing. She also obtained an associate’s degree in Science at Peirce Junior College in Philadelphia. “It is with great enthusiasm and pleasure that I am able to assist our employees and their families with their medical concerns,” Pascale says. “I am excited to get started.”

Their experience and dedication to Crozer-Keystone Health System is beneficial in helping employees achieve their health goals on the ‘road to wellness,’ says William Kreider, vice president of Human Resources for Crozer-Keystone Health System.
“Road to Wellness” participants are eligible for rewards, including the best reward of all — the opportunity to maintain or improve their overall health and quality of life. Where do you start? It’s easy. Go to http://wellnessworks.crozerkeystone.org and click on “New Members Start Here.” Enter the sponsor code “CKHS” and then “submit” and follow the instructions for a health assessment.
Next, watch for the free “Know You Numbers” biometric screening dates and times at each Crozer-Keystone hospital and schedule you appointment online. Additional information will be mailed to eligible employees’ homes and is available online at http://crozerbenefits.org.

The nurse navigator works closely with you and your physician to take action to improve your overall health and quality of life, particularly those suffering from chronic conditions.
