Flu Vaccination: A Notice to Crozer-Keystone Physicians, Employees and Volunteers
Getting vaccinated for influenza not only protects you, but also reduces the risk of infection to your co-workers, your patients, and your families.
Our system-wide employee vaccination program began in September. All CKHS employees and physicians may receive the influenza vaccination free of charge.
This year, CKHS has decided to make mandatory that all employees, residents, physician extenders, volunteers and physicians either receive influenza vaccination or complete a declination form indicating his/her reason for refusing vaccination.
Note that this policy is different than that of some other area hospitals: CKHS is not at this time mandating influenza vaccine. What is required, however, as a condition of employment and/or as a condition of medical staff privileges, is documentation of influenza vaccine or completion of the declination form.
Adherence to this policy will be monitored through hospital departments and the medical staff offices, in conjunction with the offices of Human Resources and the Centers for Occupational Health.
The deadline for vaccination or declination documentation is Nov. 15, 2010. Employees and physicians who do not have evidence of vaccination or declination form completion will not be permitted to work or see patients in CKHS facilities.
CKHS will make special efforts to insure wide availability of opportunities for vaccination.
