Crozer-Keystone ‘Choices’ Benefits Program Supports Good Vision Care

A good way to maintain healthy vision is to see an eye care professional on a regular basis. Most adults without symptoms need an eye exam every two to four years; those with special risk factors require them more often.

It is important to see an ophthalmologist or optometrist regularly because not only do they provide a comprehensive exam, but they also screen patients for diabetes, high blood pressure, eye tumors, macular degeneration, vasculitis and retinal detachment.

The Crozer-Keystone Choices benefits program makes it easy for employees to stay on top of their vision through the Davis Vision Plan. Coverage includes eye examinations with an ophthalmologist or optometrist, eyeglasses and contact lenses with little to no co-pay every 12 months. At present, 94 percent of employees enrolled in the plan maximize their benefits by using a Davis Vision participating provider for their routine eye care.

Davis Vision also provides you or your eligible dependants with the opportunity to receive additional savings for non-covered services from an in-network provider every calendar year. You must present your Crozer-Keystone Preferred Pricing Plan ID card to obtain savings. 

Benefits include a membership and access to a mail order replacement contact lens service, Lens 123, a fast and convenient way to purchase replacement contact lenses at significant savings. The plan also provides a one-year eyeglass breakage warranty at no cost as well as an opportunity to receive Laser Vision Corrections Services at discount rates up to 25 percent off a participating provider through a Davis Vision provider, or at significant savings through a participating Crozer-Keystone provider. Davis Vision has more than 30,000 nationwide providers. Seventy-two percent serve members in private practice settings and 28 percent are in retail locations. “With open enrollment coming up in the fall, Davis Vision is a good investment for you and your family,” Zegar says.
