Patient Privacy

We believe in your right to keep your health information private and safe. HIPAA, the Health Insurance Portability and Accountability Act, is a federal law that became effective on February 17, 2010. Under HIPAA, we must provide you with a copy of our Privacy Practice Notice (good one is second) (the “Privacy Notice”).

The Privacy Notice gives you information about:

• How we may use and share your health information
• Your privacy rights

Crozer-Keystone created a Patient Privacy Notice Summary to help you understand the most common items. A copy of our Privacy Notice and summary is given to each patient upon registration and admission to the hospital.

Hospital Directory

General information, such as a patient's room number and general condition, is available to family and friends (only for patients who are having a procedure done at the hospital or who require an overnight stay). This general information will be available to visitors and callers who ask for the patient by name, unless the patient prefers that it not be shared.

Patients can ask that we not make this general information available. If the patient asks us not to make this general information available, it is important to understand that hospital staff will not be able to:

• Give any information to the patient's family or friends who call or visit
• Direct any deliveries of flowers or cards sent to the patient while he or she is in the hospital
