Road to Wellness Physicians Resource Guide

An Important Message from Joan Richards, CKHS President and Chief Executive Officer

Dear Physicians, 

Crozer-Keystone Health System is pleased to announce the expansion of our employee wellness program, Wellness Works. Our Road to Wellness initiative provides support, assistance and opportunities to you and to Crozer-Keystone employees and family members who are your patients. These patients have medical coverage in one of our CHOICES Medical Plans – the CKHS Advantage, Advantage PLUS or Indemnity Medical Plans. 

Please refer to the attached Physician Resource Guide to learn more about Road to Wellness, particularly the Nurse Navigators. We’re counting on you to partner with our Nurse Navigators, Dee Pascale, RN and Cathy Petrucci, RN, MSN, who will work closely with you to encourage your eligible patients, particularly those with chronic illnesses, to adhere to your treatment plan. Our Nurse Navigators will provide services in an unobtrusive way so that they do not impose on you or your practice. 

Your ideas and referrals are welcome! I encourage you to refer patients, particularly those with health challenges, to our Road to Wellness program and the personalized care of our Nurse Navigators.
