Set Up Your Transamerica Account Online
First-time users will create a user name and password. When you enroll, you will select the flat dollar amount or a percent of your pay that you will save and you will determine how to invest your pre-tax savings. Be sure to name a beneficiary(ies) when you set up your account.

Your Current Fidelity or TIAA Account
Active CKHS employees are encourage to click here for information regarding any action you have to take related to your CKHS 401(a) (Defined Contribution Retirement Plan), 403(b) (Tax Sheltered Annuity/TSA), or 401(k). Carefully review this Guide to ensure you choose the right distribution option for your Plan benefits and take the appropriate actions to process your distribution in a timely manner. We also encourage you to review this Guide with your tax and/or financial advisor. The final date that you have to take action has not yet been determined; we anticipate that the deadline will be on or about Dec. 31, 2016.

- October 11: Be sure to enroll by Oct. 11! If you do not enroll by Oct. 11, deductions equal to 4% of your gross pay will automatically begin - even for non-benefit eligible employees. If you do not want to save 4% in a Vanguard Target Date Fund, you must opt out or choose a different amount.
- Future date to be determined: That will be the last day that you can take action related to your current pre-tax retirement savings with Fidelity or TIAA-CREF. You still have time! Click on the link above to refer to the booklet that explains your options (The Participant’s Guide to Plan Terminations), which was also mailed to your home.
CKHS Tax Sheltered Annuity (TSA) and Defined Contribution Retirement Plan Contributions
January to June (six month) 2016 CKHS Tax Sheltered Annuity (TSA) and Defined Contribution Retirement Plan contributions were deposited into eligible employee’s accounts. Eligible employees received one-half of the annual amount due now and Prospect will make the contribution for the period of 2016 following the Prospect Crozer LLC Plan adoption which is Sept. 1, 2016.
